We provide independent, MD physician-authored medical nexus letters for Veterans and federal employees who have been asked to document a medical connection between a diagnosed condition and military service, service-related exposures, or occupational requirements. Each medical nexus letter is prepared by a licensed MD physician and reflects an independent medical opinion. These medical opinions are commonly requested as part of VA disability and other federal disability determinations.


All opinions are based on a comprehensive review of medical records, relevant service history, applicable medical literature when appropriate, and the physician's clinical judgment. Our role is to provide an objective medical opinion. We do not provide advocacy, coaching, or guarantee outcomes.

Occupational & Environmental Medicine Nexus Letters

We offer medical nexus opinions for conditions that may be related to occupational or environmental exposures encountered during military service or federal employment. 


These opinions are informed by training and experience in occupational and environmental medicine and toxicology, with attention to both acute and long-term health effects of hazardous exposures. 


Examples of exposures and conditions that may be addressed include, but are not limited to:


  • Burn pits, AFFF, PFAS, fuels, solvents, and industrial chemicals
  • Respiratory conditions
  • Gastrointestinal disorders
  • Liver disease
  • Neurologic symptoms
  • Certain cancers (case-dependent)


Each case is evaluated individually to determine whether a medically supportable connection can be established.

Addiction Medicine Nexus Letters

We provide medical nexus opinions involving substance use disorders and related medical conditions when supported by the medical record and current medical evidence.


These opinions may address whether a substance use disorder, or its medical or functional consequences, is medically connected to factors such as service-related trauma, injury, chronic pain, or prescribed medications.


As with all our services, opinions are independent and evidence-based, and acceptance of a case depends on the strength and consistency of the medical documentation.

General Medical Nexus Letters

In addition to occupational, environmental, and addiction-related opinions, we also provide MD physician-authored medical nexus letters for common medical conditions when supported by the record and current medical understanding. These medical nexus letters may be used to support a service connection medical opinion when appropriate.


These may include, but are not limited to:


  • Degenerative joint or spine disease
  • Obstructive sleep apnea
  • Musculoskeletal conditions
  • Chronic pain syndromes
  • Cardiometabolic conditions
  • Other chronic medical diagnoses


Each case is reviewed on its own merits. We may decline cases in which a medically supportable connection to service cannot be reasonably established.


Medical Opinions for FEderal disability accommodations

We also provide medical opinions requested by individuals, employers, disability program managers, or federal agencies to address functional limitations and the medical necessity of workplace accommodations. 


These opinions focus on medical findings and functional impact, and do not constitute employment or legal advocacy.

How our nexus letters are prepared

Our medical nexus letters are written with an understanding of how medical opinions are reviewed in federal disability determinations. Each letter typically includes:


  • Identification of the diagnosed condition
  • Review of relevant service and medical records
  • Discussion of medical reasoning and, when appropriate, medical literature
  • A clear and reasoned medical conclusion expressed using standard medical probability language ("at least as likely as not"). 


All opinions are independent and reflect the physician's professional judgment.
