This is not a standard VA nexus letter. A traditional nexus letter addresses whether a medical condition is related to military service for VA disability purposes.
A military medical fitness opinion is different. This specialized independent medical opinion addresses whether your condition was severe enough, at the time of separation, to interfere with reasonable performance of duty and whether referral for disability evaluation or medical retirement should have occurred.

The goal is to provide an evidence-based independent medical opinion grounded in the documented record.
You submit the relevant records for review. These may include service treatment records, post-service medical records, VA decisions, imaging, specialist evaluations, deployment records, performance records, separation documents, and your drafted brief if applicable.
Your records are reviewed in detail by our physician. The review focuses on the medical facts, timeline, functional limitations, and consistency of the record.
If medically supportable, a written opinion is prepared addressing whether the condition more likely than not rendered you unfit for continued service at or before separation.
For more complex cases, a physician consultation may be added to discuss the case, clarify the record, and help you understand the strengths and limitations of the medical evidence.

Review of records is limited to 150 pages relevant to claimed condition and time period surrounding military service and separation.
Best for: straightforward cases with reasonably organized documentation
This includes everything in IMO-1, plus:
Review of records in this level is increased to 200 pages. Summary is focused on medically relevant events related to condition under review and is not a summary of all medical records.
Best for: more complex cases, longer timelines, or cases that benefit from a clearer written summary of the evidence.

Best for: high-complexity cases, rebuttal preparation, or cases involving extensive records and multiple medical issues
